How do I self enroll in the SMC Hospital Patient Portal?

To self enroll in the SMC Hospital Patient Portal your email address must already be a part of your SMC medical record and you will need your medical record number. Call Health Information Services at 405-742-5737 to add an email to your record or to obtain your medical record number.
• Click on “Enroll” on the SMC Hospital Patient Portal Page.
• Complete the Enrollment Request Screen and follow the prompts.
• You will receive an email with a temporary login/password and a link to complete your enrollment in the portal.

Some of my information is wrong. How do I correct this?

If you notice an error in your personal information, such as address and phone number, please send us a change request through the portal.

How do I communicate my medical concerns?

The Stillwater Medical Center Patient Portal is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 OR YOUR PHYSICIAN'S OFFICE IMMEDIATELY.

How do I see my test results?

Your test results and reports are both available by clicking on the Health Record button on the portal home page. Once you click on the Health Record button you will be taken to the Health Record page where you will see a button on the right labeled "Results" where you will have access to your Laboratory Results and a button labeled "Reports" where you will have access to general radiology reports. NOTE: Laboratory results and radiology reports are available 24 hours after they are resulted.

Can I also see my family members health records?
You may request a portal account for yourself or your minor children. Access to your child’s portal account will automatically cease when the child reaches 18 years of age, the age of majority in Oklahoma. If your child desires to continue this Proxy access as an adult, they will need to complete a Proxy Request Form and an Authorization to Disclose Protected Information allowing us to share their medical information with you.
For adults, the patient can complete a Proxy Request Form to set you up with Proxy access. If you have legal authority over an adult patient, you can request this Proxy Access on the patient’s behalf. You will be required to provide documentation of this authority (for example, a durable power of attorney) before you can receive access to that person’s health information. This must be done in person in the Health Information Services Department at Stillwater Medical Center.
